Just as a nice smile can act as a powerful communication tool, an unpleasant one can have an equally powerful negative impact that s why patients seek orthodontic treatment. As society raises the bar on an acceptable smile, we have more patients reporting for smile make-over job at our Dental Center. But Gummy Smile is one aspect which may still vex up.

In this article we are giving you an insight about gummy smile and its treatment modalities available at Delhi Dental Center — to get a desirable, predictable and achievable result.

As society raises the bar on an acceptable smile, we at our Delhi Dental Center are having more number of patients reporting to get smile makeovers by correcting there gummy smiles .

GUMMY SMILE :

The gummy smile is a condition known by an excessive display of maxillary gingival during smiling. 2 mm or more of maxillary gingival exposure while full smiling can be treated as gummy smile.

Gummy smiles are classified according to the amount of gum tissue that shows in relation to how much tooth structure is displayed while smiling. These classifications are determined by measuring the patients gum tissue as a percentage of total tooth length.

Gummy smiles ranges from mild, moderate, and advanced, to severe gummy smiles.

MILD GUMMY SMILE If the amount of gum tissue shown while smiling is less than 25% of the entire length of the teeth, it is considered as mild gummy smile.

MODERATE GUMMY SMILE If the amount of gum tissue shown while smiling is between 25% – 50% of the entire length of the teeth, it is considered as moderate gummy smile.

ADVANCED GUMMY SMILE If the amount of gum tissue shown while smiling is between 50% – 100% of the entire length of the teeth, it is considered as advanced gummy smile.

SEVERE GUMMY SMILE If the amount of gum tissue shown while smiling is more than 100% of the entire length of the teeth, it is considered as severe gummy smile.

ETIOLOGY :

1. Vertical maxillary excess, skeletal and dental.

2. Short anatomical upper lip.

3. Hyper functional upper lip elevator muscles.

4. Gingival hypertrophy.

5. Short or insufficient clinical crown length.

6. Combination of any of above.

TREATMENT PLAN —

Once the diagnosis is established and etiology becomes apparent there are two treatment options : –

1. Orthodontic option.

2. Surgical option. Periodontal.

3. Surgical crown lengthening.

4. Botox.

5. Le-fort 1 osteotomy.

1. ORTHODONTIC OPTION —

* Intrusion of teeth will help to reduce gummy smile and proclination of teeth, as the teeth intrude, gums also follow the intrusion but not at the same rate.

* Developing vertical maxillary excess may be corrected by Appling orthodontic forces with help of a head gear.

2. SURGICAL OPTION —

* PERIODONTAL — It includes simple gingivectomy up to about 2mm.

* Surgical crown lengthening — When there is short clinical crown in combination with increase gingival display, it can be corrected with removal of crestal alveolar bone.

* Botox — Increase gingival display due to hyper active muscle may be corrected by botox, the upper lip becomes flaccid and falls down there by decreasing gingival display.

* Orthognathic surgery– Sever cases of 8mm or more gingival display are best corrected by this method.

CONCLUSION —

Patient cases reporting to our center for gummy smile correction usually doesn t fall purely in any of above category. Overlapping etiological factors may contribute to difficulty in analyzing the case. However the above mentioned diagnostic criteria help to arrive at a proper etiology based diagnosis followed by logical treatment planning.

Preliminary tests performed on samples taken from six villagers in the Kabanjahe District of Sumatra in Indonesia have tested negative for the deadly H5N1 Avian Flu virus.

“Investigations by the ministry of health lab and Namru, too, on August 2 and 3 on all specimens collected from the suspected cases in Kabanjahe district came up negative,” said Indonesia’s health minister, Siti Fadilah Supari.

Final test results are expected in at least seven days from the Center for Disease Control (CDC) in Atlanta, Georgia. “The World Health Organization (WHO) requires human samples to be sent to one of WHO’s six collaborative centres. So, we only need to send them to CDC Atlanta as it has worked with the U.S. NAMRU-2 lab here,” added Supari.

Supari also stated that all individuals are suffering from the “common flu.”

U.S. Defence Secretary Robert Gates warned that limiting funding for the United States efforts in Iraq could lead to more bloodshed in the Middle Eastern country. In an interview with radio talk show host Laura Ingraham, he said it might even lead to ethnic cleansing in Bahgdad and elsewhere in Iraq.

Gates’ comment followed a proposal from Senate Majority Leader Harry Reid to end most spending on the Iraq war in 2008, limiting it to targeted operations against al Qaeda, training for Iraqi troops and U.S. force protection.

Sec. Gates also said that the duration of the troop increase is not clear and that evaluating whether the Administration’s new strategy was working will have to wait till mid-summer. The Army general charged with day-to-day operations has suggested that the increased deployment may extend to early next year.

According to US health officials, New York state governor David Paterson has declared a state of emergency in the state as a result of the H1N1 swine flu outbreak.

The Associated Press news agency reports that the six-page declaration was issued because at least 75 people have died of H1N1 related illnesses in New York since April. Three have died from H1N1 related illnesses just this past week. The declaration also says that human cases of the virus are on the rise.

Paterson says he issued the declaration because “a disaster has occurred throughout New York State, for which the affected local governments are unable to respond adequately.”

The declaration will allow health officials more access to the H1N1 vaccine and the seasonal flu shot. It will also allow for an increase in the number of vaccine doses available in the state and will allow more health care facilities to administer the vaccine, including dentists and pharmacists. Schools with health centers will also be allowed to administer both vaccines.

Despite the declaration, officials stressed that there is no reason to worry. A spokeswoman for the New York State Department of Health, Claire Pospisil, said that “it [the declaration] helps us to be more prepared.”

The order came shortly after US president Barack Obama declared a national emergency last Saturday, a response to the spreading of the virus, which has now been circulated in 46 states.

Two men charged after the body of a Chinese student was stuffed into a suitcase and thrown into Auckland’s harbour on Good Friday would deny their guilt, a court was told today.

One of them appeared in Auckland District Court today to be confronted with the charge of kidnapping a 19-year-old English language student, ransom and murder. The other accused appeared on a charge of hiding evidence for the murder. They have been granted name suppression until their next appearance in court.

According to lawyers, pleas of not guilty would be entered.

The Australian federal treasurer, Peter Costello, has announced a study to compare the Australian taxation system with those of other countries. It will include overall tax levels, as well as indirect tax, income tax and company tax systems. Mr Costello says that the results are intended to inform discussion of the future of the tax system.

“The aim of the study is to provide a public document that compares Australian taxes to those in other countries. This will identify those areas where Australia leads comparable countries and those areas where it lags. It will enable a focus on the most important areas,” Mr Costello said in a media release.

The study will be lead by Mr Richard (Dick) Warburton and Mr Peter Hendy. Mr Warburton is Chairman of Caltex Australia Ltd, and has been Chairman of the Board of Taxation since its inception in September 2000. According to its website, the Board of Taxation is “a non-statutory advisory body charged with contributing a business and broader community perspective to improving the design of taxation laws and their operation.”

The Australian Democrats welcomed the study but warned that nothing would be achieved without serious reform of the tax system.

“The Inquiry announced yesterday by the Treasurer may turn out to be a step in the right direction but will be a pointless exercise if it ends up being simply a ‘desktop’ report,” said Senator Murray, Democrats Tax Spokesperson.

“The problem with Mr Costello’s approach is that in contrast to the approach taken on the GST, the New Tax System, and the Business Tax System, he has seen income tax change to be solely part of the budget process. Across the whole of the political and public policy spectrum, there is strong agreement that reform is needed, not just tax cuts, and that the income tax system needs redesigning, not just fine tuning.”

A small aircraft struck an apartment building in New York this afternoon, killing the pilot and a flight instructor. Cory Lidle, a pitcher for the New York Yankees baseball team, was the registered owner of the aircraft and is believed to have been piloting it; his passport was found on the scene. The flight instructor was Tyler Stanger.

The plane, a Cirrus SR20 with registration number N929CD, hit the 26th floor of the Belaire Condominium, a 50 story brick luxury residential building on the Upper East Side at 524 East 72nd Street at York Avenue near the East River in Manhattan, New York City, of which the first 20 floors are a hospital. An eyewitness, present half a block from the building, reported that the plane hit the building, creating an enormous fireball, broke in two and crashed down onto on the street below. Authorities received a 911 call reporting a crash at 2:42 p.m. Eastern time.

New York City mayor Michael Bloomberg gave a press conference at approximately 5:20 p.m. Eastern time to report that the plane was occupied by a flight instructor and a student pilot, but as next-of-kin had not been able to be notified, the identities of the two people on the plane would not be released at this time.

Apartments were seen to be engulfed in flames. The FBI has stated that it was not an intentional attack. The New York area was grey and overcast during the time at crash; however, visibility was not hampered.

After an hour and a half, the fire was extinguished by the FDNY. According to CNN Television ten people have been injured, six of them firefighters. The New York Times, however, reports that eleven firefighters have been injured.

Initial reports suggested that a helicopter was involved, but the FAA has stated that it was a fixed-wing aircraft. CNN Television reported that it was a single-engine fixed-wing plane which left Teterboro Airport, a busy General Aviation airport in New Jersey, circled the Statue of Liberty was tracked on radar until it was lost near the 59th Street Bridge, that the plane may have been having fuel problems or fuel pump problems, and that it was “a pilot in distress.”

Wallace Sines, a source for CNN stated he believes the plane was a Cirrus SR-20 with an installed parachute, which did not deploy. The whole-plane parachute system may have saved the lives of the aircraft occupants had it been safely deployed clear of buildings, but the system is not designed to prevent the trauma associated with a plane impacting a builing. The Cirrus SR-20 was introduced in 2001 and the Cirrus line of 4-seater aircraft has since become one of the most purchased single engine aircraft in the world. The SR-20 does not normally carry an airline-style flight-data recorder, but some are equipped with GPS equipment which logs flight direction, speed and altitude.

The aircraft was owned by baseball player Cory Lidle. He was on board reported by AP. Lidle was killed, according to reports. CNN Television reported that the FBI stated he was at the controls as the only occupant of the plane, and that his passport was found on the ground below the accident.

A little over an hour and a half after the crash, the fire was extinguished after 39 fire units and over 100 fire fighters responded.

The White House has said that there has been no change in the terror threat alert level and that President Bush is being updated constantly.

La Guardia airport was temporarily restricted to no take offs from other airports, however by 4:10 Eastern Time, CNN Television reported that all New York-area airports were open.

CNN Television announced at 3:50 p.m. Eastern Time that as of a few minutes earlier, NORAD is putting fighter aircraft on patrol over certain major American cities as a precautionary measure similar to the actions taken after the 9/11 attacks as a “just in case” measure. It also reported that tomorrow is the 6th anniversary of the attack on the USS Cole in Aden, Yemen.

Moments after the crash, there was a drop in the New York Stock Exchange, however it quickly returned to normal after it was reported that the crash was an accident.

The New York Yankees organization confirmed the plane is registered to Cory Lidle of the New York Yankees, who was planning to fly from New York to Florida, and that some unnamed member of the Yankees organization was on the plane.

One eye-witness interviewed by the BBC stated: “I was wondering why the plane was doing acrobatics and then the next thing I knew was that it had crashed into the building.”She also added that the plane was a small, white, 4-seater winged aircraft and not a helicopter as many news agencies were reporting it to be.

CNN Television broadcast reports from eyewitnesses who reported:

  • A pilot who saw the impact stated, “It looked like a pilot who was desperately trying to get to an airport.”
  • Another eyewitness who saw the event from the Brooklyn-Queens Expressway, also referred to the incident as appearing as if the plane was “desperately trying to get to LaGuardia” and that as to whether he “clipped” the building, or struck it directly, that “he hit it dead on.”
  • “I heard a buzzing noise and then an explosion which looked like a mushroom cloud.”
